Strict handling of connection error

Following cases are now treated as connection error of type
PROTOCOL_ERROR.

* Receiving HEADERS from client in reserved (local)
* Receiving PUSH_PROMISE against peer-initiated stream
* Receiving WINDOW_UPDATE against in reserved (local, remote)
